New Patients


If you wish to become a patient of the practice, you will need to contact the practice by telephone or in person to make a New Patient Appointment.

You will need to complete a new patient questionnaire and a confidential medical history questionnaire. This will provide us with essential information to enable us to treat you in the safest way. If you are unsure of your drugs, please contact your pharmacy or doctor for a full list (a print out is very useful).

These forms may be sent to you in the post or collected from the practice and should be handed in at the time of your appointment or before.

In keeping with our preventive approach, we prefer to look after the complete dental needs of our patients.

However, we understand that not everyone is looking for a comprehensive service and may be seeking to have a specific treatment carried out.

We are happy to oblige wherever possible, so don’t hesitate to contact the practice with your individual needs. Examples may be as follows:


We will need to make an assessment prior to undertaking the treatment. This can sometimes be done in the same appointment.

If you wish to see one of our hygienists, but are a patient at another practice, we are required to request a referral and treatment plan from your own dentist. In this way, you would not necessarily need to see one of our dentists first.
